[Cin] Cinelerra context help via HTML manual

A working example, how such a HTML manual, without much additional efforts,
can be implemented to get CinelerraGG context help (here: context help for
all the audio and video plugins, after minimal changes in a single Cinelerra
source file) - I have described on Bugtracker as BT540, with the working
patch.
